Proof of Vaccination for Those Without Health Card Numbers

The following link can be used to schedule an appointment to have your receipt printed if you do not have a healthcard or did not provide your healthcard number. If you require assistance please call 226-773-2200.

You will need to bring two (2) of the following pieces of identification to your appointment:

  • Government issued photo identification (e.g., drivers license, health card, passport)
  • Birth certificate
  • Citizenship card
  • A piece of mail with your name and address
  • A letter from your employer verifying your name

Why?
  • Delta, Lambda, & Mu variants
  • Increased infection rates
  • Higher risk of transmission
  • Enhanced protection for those fully vaccinated
  • Keep businesses open
  • Foundation capability
When and How?
Who? (non-essential services)
  • Higher risk indoor public settings
  • Restaurants and bars (excluding outdoor areas)
  • Nightclubs (including outdoor areas)
  • Meeting and event spaces (banquet halls conference centres)
  • Sports and fitness facilities
  • Sporting events
  • Casinos, bingos and gaming establishments
  • Concerts, music festivals, theatres and cinemas
  • Racing venues
How?
  • Currently – paper, digital, laminated with ID
  • October 22 – new app that allows you to display a QR code
  • New process being established for those that have no email or health card
  • Enhanced vaccination certificate through QR code will be scanned
  • Aimed at protecting privacy
  • Avoids retention of information
Exemptions
  • Access to medical care, food, basic medical supplies or essentials
  • Voting
  • Short exemption window provided to allow for vaccinations (i.e. indoor funerals / weddings)
  • Limited medical exemptions
  • Under age of 12
  • Accessing a washroom
  • Paying for an order
  • Picking up or placing an order

Negative COVID test or recent infection does NOT preclude you from providing proof.

Employers
  • Reasonable or Unreasonable Request by an employer / service provider
  • Not required in office settings
  • Policy recommended
